The Bricasti M3h could serve as an all-in-one streamer/DAC/headphone amp/pre-amp. There seem to be many great reviews for Bricasti. There's also the upcoming Boulder 812 streamer/DAC/headphone amp/pre-amp that could fit the bill too. The Matrix Audio Element X is another, more cost-friendly option that checks all the boxes.
